NOTICE IS HEREBY GIVEN that Ashanah Barnes, pursuant to Wyo. Stat. §2-1-205, has filed a sworn Application for A Decree of Distribution in the District Court, Third Judicial District, County of Uinta, State of Wyoming stating:

  1. That Emma Darlene Powell and Steven Verle Powell (Husband and Wife) passed away simultaneously on the 8th day of September 2021 in a Motorcycle vs Motor Vehicle head on collision in Sweetwater County, Wyoming.
  2. Said Application alleges that Emma Darlene Powell and Steven Verle Powell were tenants by the entireties in the property described herein at the time of their simultaneous death in Sweetwater County Wyoming.
  3. Street Address 724 Uinta Street Evanston, Wyoming.
  4. WHITE’S ADD BLK 4 L T 8 PT L T 9 BECKWITH’S 1 ST ADD BLK 4 L T 8 PT
  5. That less liens and encumbrances, the entire estate of each decedent is less than $200,000.00.
  6. That pursuant to Wyo. Stat Ann. 2-13-105, the property shall be distributed one-half (1/2) as if one (1) had survived and one-half (1/2) as if the other had survived.
  7. That Ashanah Barnes is the sole beneficiary and devisee entitled to the property pursuant to a joint Holographic will left by decedents.
  8. Said application requests that this court issue a Decree of Distribution, distributing said property to Ashanah Barnes.

Any objection to said Application must be filed in said Court within thirty (30) days of the first date of publication. If no timely objections are filed, the Court shall enter the requested Decree establishing the right and title to said real property.

PUBLIC NOTICE

Pursuant to the Wyoming Administrative Procedure Act and the Wyoming Public Service Commission’s (Commission) Rules, notice is hereby given of the Application of Questar Gas Company d/b/a Dominion Energy Wyoming (Dominion or the Company), for approval of the 2022 year budget for Energy Efficiency Programs.

  1. Dominion is a public utility as defined by Wyo. Stat. § 37-1-101(a)(vi)(D), subject to the Commission’s jurisdiction pursuant to Wyo. Stat. § 37-2-112.
  2. On October 29, 2021, Dominion submitted its Application requesting approval of its 2022 year budget and modifications to the ThermWise® Energy Efficiency Programs pursuant to Wyo. Stat. § 37-2-121, Commission Rule Chapter 3, Section 21, and the Commission’s Order issued on February 6, 2013, in Docket No. 30010-119-GT-12 (Record No. 13321).
  3. Dominion proposes continuing the five existing ThermWise® Energy Efficiency Programs (Programs): [i] Appliance Rebates Program; [ii] Builder Rebates Program; [iii] Business Rebates Program; [iv] Home Energy Plan Program; and [v] Weatherization Rebates Program.
  4. In addition to making minor tariff language changes to improve accuracy in each of the ThermWise® Programs’, the Company proposes 2022 additions of: residential heating ventilation and air conditioning monitoring and diagnostic systems in both the Appliance and Builder Rebates Programs; rigid foam exterior insulation to the Weatherization Rebates and Builder Rebates Programs; and two categories of advanced boiler control systems linkageless controls and O2 trim controls in its Business Rebates Program in 2022.
  5. Dominion projects the total 2022 budget for the Programs and Market Transformation Initiative to be $167,868, an increase of 3% from the 2021 budget. Dominion states the proposed budget increase is primarily due to greater forecasted 2022 participation in the Appliance, Builder, and Business programs than were projected in 2021. The Company projects a total natural gas savings at 6,256 Dth per year, 7% above its projected 2021 natural gas savings estimates.
  6. This is not a complete description of the Application. Dominion’s Application is on file at the Commission’s offices in Cheyenne, Wyoming, and is available for inspection by any interested person during regular business hours or online at: http://psc.wyo.gov.
  7. UINTA COUNTY SCHOOL DISTRICT NUMBER ONE

REQUEST FOR PROPOSAL

TO PROVIDE HEALTH INSURANCE CONSULTING SERVICES

U.C.S.D. Number One is interested in receiving proposals from qualified health and benefit insurance agent/agency to provide consulting services concerning the U.C.S.D. Number One group insurance products. 

U.C.S.D. Number One Administration is required to make recommendations concerning what insurance products are provided, benefit levels, and selection of insurance providers.  U.C.S.D. Number One Administration also reviews insurance utilization and cost, and has the responsibility to periodically bid group insurance products and review proposals from benefit providers.

The agent/agency/consultant will be highly involved in assisting the U.C.S.D. Number One Administration in fulfilling these functions.

Scope of Services

  1. Negotiate rates and benefits with insurance providers, subject to approval by U.C.S.D. Number One.
  2. Prepare specifications and assist U.C.S.D. Number One in bidding group insurance products. This includes preparation of RFP’s and evaluation of bids or proposals.
  3. Provide U.C.S.D. Number One with consulting services concerning cost, liability, benefit structure, and contracts with providers for current insurance benefit package.
  4. Prepare reports concerning trends, utilization, and costs.
  5. Recommend benefit changes to meet the objectives of U.C.S.D. Number One.
  6. Be the advocate for U.C.S.D. Number One and individual employees in working with insurers to ensure compliance with insurance contracts and in dispute resolution with benefit providers.
  7. Assist U.C.S.D. Number One in the containment of benefit costs.
  8. Agent/Agency will be responsible for all COBRA notices required by law. A COBRA administrator may be contracted at the Agent/Agency expense, but all federally required notices on the employees behalf are the responsibility of the Agent/Agency
  9. Other duties as requested.

Terms of Contract

It is anticipated that the U.C.S.D. Number One will enter into a contract with the successful bidder that will be effective January 1, 2022 through December 31, 2025.

The initial term of the contract will be for 3 years with the option of renewal for additional one-year terms at the discretion of U.C.S.D. Number One.  This contract may be canceled at any time within 30 days of written notice from either U.C.S.D. Number One or the agent/agency.

The contract will require the consultant to work on a monthly fee basis

Proposal Content

  1. An explanation of the agent/agency’s experience in providing insurance consulting services for major companies, governmental entities or school districts.
  2. A list of major clients for which the firm has provided consulting services. For each client please list the services provided and insurance companies utilized.
  3. Include resumes of individuals who would be assigned to work with U.C.S.D. Number One if your firm is selected as our insurance agent/agency. Describe the role that each individual would play.
  4. Identify if agent/agency is licensed in Wyoming.
  5. Outline unique services that qualify your firm to serve U.C.S.D. Number One.
  6. Describe your experience with the benefit providers currently utilized by U.C.S.D. Number One. Please make suggestions concerning modifications you would recommend to current benefits.

Selection Process

2, After evaluation of the proposals, finalists will be asked to make a presentation to the U.C.S.D. Number One Administration/Board.

  1. Final selection will be based on U.C.S.D. Number One assessment of the written proposal and interview.

U.C.S.D. Number One reserves the right to reject any or all proposals and to waive any informality or technicality in any proposal.

  1. Reputation and expertise in providing insurance consulting among similar clients    20%
  2. Ability to negotiate rates and benefits 20%
  3. Understanding of U.C.S.D. Number One products and suggestions for future considerations           30%

Agent/agency are required to mark any specific information contained in their proposal, which is not to be disclosed to the public, or used for the purposes other than the evaluation of the proposals.  Pricing and service elements of the successful proposal will not be considered proprietary.  All materials become the property of U.C.S.D. Number One and may be returned.  Proposals submitted may be reviewed and evaluated by any person at the discretion of U.C.S.D. Number One.
